Important reminder: Notify licensing agency if contact info changes
Is your contact information up to date in the records of the Indiana Medical Licensing Board (MLB)? If your information has changed and you have not notified the MLB, you might be missing important communications, such as a notice that your medical license will expire on Oct. 31, 2021, and information on how to renew it in time. 

Hoosier physicians are required to notify the MLB within 20 days if their address changes (see 844 IAC 4-4.5-19). This is especially important since 2021 is an odd-numbered year, in which all Indiana medical licenses will expire on Oct. 31 if they are not renewed. Under Indiana law, once the Board has mailed a renewal notice to the address of record, it has discharged its duty and is not responsible for a physician’s failure to renew a license on time or for denial of a license if a physician has failed to update their address records as required (see 844 IAC 4-6-2.1). 

It’s easy to update your business, personal and email addresses at the same time you renew your medical license or by logging in here. Updating your email is especially important, to be sure you receive updates from Indiana’s prescription drug monitoring program, INSPECT, as well as from the MLB.

A change of address that relates to your Controlled Substances Registration (CSR) and Drug Enforcement Administration (DEA) registration must be done separately. A change of address for your CSR must be reported to the MLB here. Updating address information for your DEA registration can be done by using the online DEA registration tool.
