• Smoking Cessation Provider Initiative Program: This program incentivizes providing smoking cessation counseling to members who use tobacco, including referring them to Indiana’s Tobacco Quitline. It will offer an incentive on paid professional claims, in addition to the reimbursable amount for the service to practitioners who meet the requirements.
• Physical Health Provider Experience Managers: This map shows names and territories of Anthem’s Indiana Medicaid provider experience managers.
• Network Relations Rep Territory Map: This map shows Anthem’s network relations managers’ names and territories.
